Decoration: Fols. 1v and 2r have full illuminated borders with foliate motif; 17-line historiated initial on fol. 1v, depicting an Augustinian presenting a book to Pope Martin V; 18-line initial on fol. 2r, depicting an Augustinian reading from a podium to a small audience; arms of Colonna family in bas-de-page. 9-15-line historiated initials, with illumination, at major breaks, depicting the Augustinian, the Virgin Mary, and/or the Christ child. 7-9-line illuminated initials at head of questions, 3-4-line pen-flourished initials alternating red and blue throughout. Question numbers in running header.
Origin: Luca, lector of the Augustinian house at Offida, commissioned this codex as a presentation copy for Pope Martin V; the manuscript was written by Iacobus Nycolay in central Italy in 1419, probably in the Marche region, possibly in Offida. See colophon fol. 298v: "Finitum et scriptum est hoc opus per manus Iacobi Nycolay et alia prededencia huic operi de alma ... ad peticionem et requisicionem ... fratris Luce lectoris de Offida ... arte pre aliis splend. ..." In red, following the colophon, "Qui scripsit scribat semper cum Domino vivat vivat in celo cum Sancto Bartolomeo."
